Acknowledgement of Risk: By participating in the Family Fun Fishing Day hosted by Columbia Sportswear South Africa at Gnu Fly Fishing, I acknowledge that fishing involves inherent risks, including but not limited to the risk of injury, drowning, bites, hooks, equipment-related injuries, and environmental factors (such as weather, terrain, or wildlife). I understand these risks and voluntarily agree to assume all responsibility for myself and my dependent child while participating in the event.
Waiver of Liability: I hereby waive and release Columbia Sportswear South Africa, Gnu Fly Fishing, their employees, agents, contractors, and any affiliated organisations from any liability for personal injury, loss, or damage to property resulting from my participation and my child’s participation in the event. This includes any harm that may arise from the use of the venue, equipment, or failure to follow instructions.
Medical Conditions: I confirm that both my child and I are physically fit and able to participate in the Family Fun Fishing Day. If either of us has any medical conditions or special requirements (such as allergies, injuries, or the need for medication), I will inform the event organisers in advance. I agree to carry any necessary medication (such as an inhaler or epinephrine) and to take full responsibility for managing both my own and my child’s health during the event.
Underage Participants: I acknowledge that my child (under 18) is participating in the event under my supervision and full responsibility. I understand that I am required to be present with my child during the event and ensure their safety, behaviour, and adherence to all event guidelines.
Use of Photographs and Video: I understand that photographs and videos may be taken during the event for promotional purposes, including social media, websites, and other marketing materials. By participating in the event, I consent to the use of any images or videos that may feature me or my child.
Indemnification: I agree to indemnify and hold harmless Columbia Sportswear South Africa, Gnu Fly Fishing, their employees, agents, contractors, and affiliated organisations from any and all claims, demands, or causes of action arising from my participation and my child’s participation in this event, including any claims made by third parties.
I acknowledge that I have read and understood this Indemnity Form, and that I voluntarily assume all risks associated with the Family Fun Fishing Day hosted by Columbia Sportswear South Africa at Gnu Fly Fishing. I agree to abide by all safety guidelines and event instructions and take full responsibility for my child’s participation.
